The Dayton Pirates are proud of their storied athletic traditions. Over 50% of students participate in a school sport at Dayton High School.

The following must be completed prior to practicing and competing in Dayton Athletics. Copies may be picked up in the Athletics Office, or printed by clicking the links below. Please be sure to fill out the forms completely and accurately before turning them in to the Athletics Office. 

Required Forms for Athletics Participation

Emergency Contact Form - Emergency Contact for Sports. Must be completed during registration.

PHYSICAL EXAMINATION FORM  or EXAMEN FISICO  - Must be completed by a physician.
